This week in clinic the Mirena IUD has come up quite a bit. It’s a super common form of hormonal birth control that has some mixed reviews. Compared to its older sister the copper IUD, it’s often recommended as a better option for gals with heavy/painful periods. It’s commonly considered convenient, cost effective and can actually be helpful for some women who suffer from the above menstrual symptoms. While some women love it, others find it doesn’t jive with them at all! Either way it stands to reason that the varied experience can be attributed to how the synthetic hormone in the Mirena affects each woman individually.

The Mirena contains a synthetic hormone called, levonorgestrel, a progestin that slowly releases over time. Progestin thins the lining of the uterus, so a fertilised egg cannot implant. This is why it’s common for women to have lighter periods. Progestin also stops ovulation by suppressing FSH and LH which work together to mature an egg and trigger ovulation, while also signalling the rise and fall of hormones. It also thickens secretions to make it difficult for sperm to make it to the egg if ovulation does occur.

It all sounds convenient, but truth be told when synthetic hormones are released in the body they do not act like actual hormones and can lead to imbalances, especially if you already have issues in that department.

Here are some possible side effects with hormones in mind:
- Missed periods while it’s in and sometimes taking a while to come back after it comes out. Some women end up with Amenorrhea (missing period – aka no ovulation!) for a while after it’s removed.
- Unpredictable periods - spotting constantly, random periods, longer periods
- Mood swings
- Depression & anxiety
- Nausea
- Ovarian cysts
- Headaches and migraines
- Bloating and gut dysbiosis
- Acne
- Hair loss

Rare, but possible side effects:
- Perforation: when the IUD embeds in your uterus
- Expulsion: When the uterus pushes it out. Women with heavy or painful periods are at higher risk of expulsion. Also, postpartum women.
- Increased risk of breast cancer
- Pelvic inflammatory disease

The Course of Treatment.

Over my 20 years in practice as an acupuncturist I have seen a steady increase in the number of people seeking out Chinese Medicine and acupuncture. Even today there is always a large proportion of patients who are new to acupuncture and are not sure what to expect from a treatment. Curiously, there are some who come in expecting some mysterious miracle cure from one session, but mostly patients understand that a course of treatment needs to be applied for a favourable clinical outcome to be achieved.

While we diagnose and treat every patient as a unique individual with specific requirements, some of the more obvious influencing factors that determine how long a treatment course will take include;

· the length of time the condition has been afflicting a person
· the severity of the condition at the time of treatment
· the ability and willingness of a patient to change aggravating factors in their diet and/or lifestyle
· a patient’s general levels of mental and physical health
· a patient’s constitutional makeup and age
· the amount of pharmaceutical medicine being taken

Generally speaking, a course of acupuncture treatment is going to require roughly 4-6 treatments (usually 1 session per week, sometime 2) to start to get on top of the issue and provide significant improvements. Often significant changes can be achieved within 1-2 sessions, but this is not always the case. Once this initial 4-6 session has been completed, treatment is usually pushed out to a fortnightly session for a short period of time, then a monthly, just to make sure relief and improvements have been sustained.

It’s usually at this time that many of our patients understand the profound benefits that maintenance acupuncture treatments can have on keeping them healthy and well. It’s not unusual for people to reports a generalised improvement in their overall health and wellness as a result of the acupuncture treatment, including better energy levels, sleep, digestion and lowered stress levels.

One of the things about Chinese Herbal Medicine is that they are not just herbs. The system has evolved over at least two Millenia and undergone constant refinement. There is a close and special relationship between the herbs within the Chinese Pharmacopoeia, and human life - so much so that the herbs even describe people and the changes that people go through. Classical Chinese Medicine herbalists “think” in terms of herbs and understand people and the changes they go through in relation to herbs.

The herbs come together in varying ratios and proportions, like little gangs, and make formulas for different purposes. Sometimes the formulas are very similar to each other, but with different ratios or slightly different ingredients, and produce different changes in the body.

Chinese Herbal Medicine is like a Deep Nutrition. It goes much beyond the kitchen and the vegetable garden, but very much needs both those things. It reaches out into the nature of nature and over a long time has collected knowledge and wisdom of the plant and animal kingdom and how it affects the human organism. We are very lucky indeed to have been handed down such sophisticated knowledge of Chinese Medicine Pharmacopoeia!

May we never forget this time when extensive research into natural alternatives is ignored and lifestyle factors are deemed unimportant when the evidence says otherwise.

These little numbers can be made in moulds or just in little cupcake cups.

1 block of Lindt dark chocolate
1 tablespoon of coconut oil
1 cup frozen raspberries
2 tablespoons of maple syrup
1 tablespoon of coconut cream

Melt the dark chocolate and coconut oil over a double boiler and pour a little bit of chocolate into the moulds, running the chocolate around the edges to make a case.
Pop them in the freezer for a moment.

Mix the raspberries, coconut cream and maple syrup together until wet and combined.

Once the chocolate has hardened pop a little spoon of raspberry filling into each mould.

Now cover with the remaining chocolate and freeze.

Voila. Delicious raspberry chocolate bites.

Did you know that as Chinese Medicine practitioners we often train in and practice various ‘styles’ of acupuncture. Within the ‘East Asian Medicine’ traditions many and varied styles of acupuncture have developed over time. Often the style and tradition developed with a family linage or under a renowned master practitioner. Even today as acupuncturists, we have the opportunity to train in and practice a number of different styles of acupuncture that may have very different approaches to acupuncture therapy.

In clinic you will come across practitioners using acupuncture styles such as TCM, Japanese meridian therapy, Manaka protocols, Kiko, Tan, Tang, Esoteric, Saam, to name a few. All of these styles have their own perspective and approach on how to apply acupuncture therapy to a patient. This will involve variations in the types of pins used, the number and location of points, the strength or gentleness of the application of the pins, the duration of time that the pins remain in the points, as well as whether adjunct therapies like moxibustion, cupping and gua sha are used.

It's our task as therapists to consider and apply the correct treatment to a patient using the particular approach and tools that our preferred ‘style’ of acupuncture affords us. We also formulate a treatment based upon a person’s constitutional makeup and the pattern(s) of disharmony that we are treating. Not only is it important for us to choose the correct acupoints to use, but we must also gauge the ‘dosage’ of that treatment applied, which includes the right amount of points and the amount of time the pins are left in place.

It’s important to communicate that with acupuncture treatment, more is not necessarily better or appropriate. While we all love the chill out time we get in an acupuncture session, please do not judge its worth based upon how many points are used, or how long you get to rest on the table. Know that your acupuncturist is taking many things into consideration when applying your individualised treatment in an effort to maximise positive clinical outcomes for you.

What really mattered was evenings spent with your friends. It’s maybe been months since you’ve seen each other but nothing ever changes.

What really mattered were the tears you shed over FaceTime. When not even distance could stop you from spending time in each other’s company.

What really mattered was dinner at the beach on a hot night. Laughter. Connection. Walking barefoot on the grass. Meeting your friends new baby for the first time. Being vulnerable with people. Your pets. Honesty. Being fearless in the pursuit of your happiness. Hand picked flowers. Wearing what makes you feel the most confident. Loving yourself. Loving others. Love.

All that really mattered was how you loved. Who you loved. And how your love made others feel.

That’s what you leave behind.
